Supported living

We provide accommodation, care and support for people with learning disabilities and mental health needs aged 18years and over. We provide personalised care for all our residents and each resident has an allocated keyworker.

The service that we will provide include the following:

  • We will support each resident to maintain their own tenancy
  • We will ensure that the residents, their families and significant others are involved in decision making about their care and are supported to do so. We believe in the notion of “no decision about me without me”.
  • We will ensure that each resident is aware of the importance of health and well-being and supporting them to register and access local GP and dentist of their choice.
  • We will ensure that each resident is aware of the importance of health and safety and are able to safely use household and kitchen appliances.
  • Assist residents with activities of daily living and helping them establish daily routine like shopping, cooking, budgeting and cleaning.
  • Monitoring the residents mental health well-being and take appropriate action to address any concerns by referring them for appropriate care and support network needed.
  • Monitoring resident’s physical health and making sure that their health needs are met including booking appointment and keeping appointments and follow up. This may involve accompanying residents for appointments.
  • Encouraging and assisting residents to access daytime activities of their choice for example, day services, volunteering, training, education and employment.
  • Assisting residents to access leisure activities of their choice including engaging in physical activities.
  • Assisting residents to adhere to any prescribed medication.
  • We work collaboratively with other multidisciplinary teams for the benefits of our residents whilst maintaining client’s confidentiality.
